Hyperbaric Health Int’l – Client Safety Acknowledgment

I,   have received the following: 

1. Acknowledgment of Training and Instruction

I acknowledge that Hyperbaric Health Int’l has provided me with:

• A complete User Manual for my hyperbaric chamber and related equipment.

• Video training materials covering operation, safety, and best-practice guidelines.

• Personalized instruction on how to equalize ear pressure during compression and decompression.

I confirm that I have read, watched, and understood these materials and that I have had the opportunity to ask questions about proper chamber use and safety protocols.

2. Agreement to Safe Operation

I understand and agree to:

• Never run consecutive sessions  to prevent the possibility of central nervous system toxicity. 

• Follow all recommended time, pressure, and oxygen protocols as provided in my training materials.

• Operate the system only in accordance with manufacturer and Hyperbaric Health Int’l guidelines.

• Immediately stop any session if I experience discomfort, dizziness, ear pain, or any unusual symptoms.

3. Health and Safety Responsibility

I understand that mild hyperbaric oxygen therapy (mHBOT) is a wellness modality, not a medical treatment, and is not intended to diagnose, treat, or cure any medical condition.

I am solely responsible for consulting a licensed healthcare provider regarding my personal health status or any conditions that may affect my ability to use this equipment safely.

4. Release, Indemnification, and Hold Harmless

By signing this agreement, I hereby release, discharge, and agree to hold harmless Hyperbaric Health Int’l, its officers, agents, employees, affiliates, and representatives from any and all claims, demands, or causes of action arising out of or connected with the use or misuse of any hyperbaric chamber or related equipment provided by or purchased through Hyperbaric Health Int’l.

I understand that Hyperbaric Health Int’l has made best efforts above and beyond industry standards to ensure my safety through training, documentation, and support. I accept full responsibility for my actions while using the equipment and agree to indemnify Hyperbaric Health Int’l against any injury, loss, or damages resulting from my operation or failure to follow the provided instructions.

5. Acknowledgment of Understanding

I have read this waiver and fully understand its terms. I acknowledge that I am signing it freely and voluntarily, and intend by my signature to provide a complete and unconditional release of all liability to the greatest extent allowed by law.
